Form and document identification can and will remain one of the most difficult items to identify. I hope this section will help in someway. I have added WWI forms as a number records remained the same in content they transmitted. If you can assist please contribute.


Army Form B.218P:

Army form B.243 and B.244:

Formal will. The witnessed will, written and signed by the soldier.

Formal will from a soldier's service record and pay book (A.B. 64) part I is a witnessed will, written and signed by the soldier on Army Form B.243 (naming his/her beneficiary) or B.244 (for more than one beneficiary). Both types allowed for the naming of executor(s).

During WW II these forms stayed in circulation. However a similar form was introduced, Army Form B.2089, and was commonly used inside pay books instead of Army Form B.243 and Army Form B.244.

Army Form B.2089:

An informal will from Army Book 64, soldier's service record and pay book (Army Book 64).
Form of will, leaving the whole of his property and effects to one person. This form is removable.

National registration identity card. This is not an army form as such. The identity card was issued to all persons until 1953?

Army Form W 3118:

Servicemen and women who drop out line during a march will be seen by a M.O. Should the M.O. consider them unfit to carry on he should give him/her written permission to use an ambulance or other available transport them.

If the M.O. considers that an individual requires admitting to a field hospital he should attach a field medical card (A.F. W 3118) describing his/her disability.
 
 
Army Form W 3149:

Medical. AB-64 Part 1 will be inspected to ensure that the soldier has had a medical inspection on AFW-3149 and that he has been vaccinated and inoculated within the time-limit. If necessary, he will be examined, vaccinated and inoculated.
 
 
Army form W 3185:

In accordance with F.R.S., Vol 1. Daily state of sick and wounded or in other matters as incidences of diseases and expenditure of materials. See above also.
 
 
Army form W 3118A:
 
WWII:
A field medical card A.F. W.3118 and envelope A.F. W.3118A should be, if possible made out for patients passing through the R.A.P. and a note of morphine and dosage recorded and the administering of tourniquet being applied.to ensure the patient receives prior treatment on arrival at the A.D.S.
